Patient Portal and Patient Forms Established Patients can now use our patient portal to view information and add information to your chart. Click on the portal below once you have received a unique ID number from our office. Established patients will be able to view their chart, check appointments, cancelling appointments, updating history and sending non-urgent messages to the office. You can register your patient information in the portal before your appointment, but you will still need to fill out the paperwork below.
Rappaport Dermatology accepts most all insurances, including CDPHP, MVP, Empire BC/BS and any BC/BS we are able to bill our local BC/BS, Fidelis, Multiplan, Medicare, Most Medicare Advantage Plans, Most Blue Shield Plans and many others. If you are unsure if we participate, please contact your insurance plan to see if Dr. I. Paul Rappaport's is participating or call our office at 518-587-9243.
We accept payments in the form of Cash, Check, Mastercard, Visa, American Express, Discover and CARE CREDIT.
