Welcome to Rappaport Dermatology, the office of I. Paul Rappaport, M.D. Rappaport Dermatology provides the latest in skin care treatments for patients of all ages and skin types, and we have been in business for over 30 years! Dr. I. Paul Rappaport services all areas of dermatology, dermatologic surgery, as well as cosmetic dermatology. Rappaport Dermatology is proud to offer the latest state-of-the-art technologies to help you look and feel your best. We would like you to know that we are dedicated to service your needs by striving for quality results. We are most interested in you and what you have to say.
Our office will normally mail you a new patient packet of paperwork or you can download forms online that must be completed and brought to your appointment. You will also be required to present your insurance identification card(s) at the time of service along with a valid photo identification card, i.e. driver's license, military card, student ID, or government picture ID to your appointment. Please note, these two forms of identification are required at the time of your visit. Without these, we will need to reschedule your appointment.
TREATMENT OF MINORS: All minors, under 18, must be accompanied by parent/legal guardian for initial visit. Subsequent visits may be accompanied by written permission from parent/guardian only for another adult over 18 years of age.
OFFICE HOURS: Monday 9:00am-2:00pm, Tuesday & Wednesday 8:30am-5:00pm and Thursdays 8:30am-1:30pm
Closed on Fridays.
INSURANCE: If your insurance requires a referral and/or coinsurance or deductibles, you will be responsible for bringing the referral and your coinsurance/deductible at the time of service, as required by your insurance carrier. If you do not bring your required referral; in order to be seen, you will be required to sign a waiver and will be responsible for the bill, unless a referral is received within 5 business days of the office visit. All non-participating insurance and self-pay patients will be responsible for their bill at the time of service. PRESCRIPTION REFILLS: When requesting prescription refills, please allow up to 24 hours to process your request. Please make available all prescription information, as well as the pharmacy that you will be using to fill your prescription.
RESCHEDULE/CANCELLATIONS: We request a 24-hour prior notification for rescheduling or cancellation of appointments, so that we may offer your appointment time to other patients. We realize there are sometimes circumstances beyond your control, but we still ask that you communicate with our office There will be a "No Show" fee of $50.00 for office visit broken appointments and $150.00 for all procedures, i.e. biopsies, shave removals, excisions, Acne IPL, acne surgery and injections without 24 hour notices or non-communication with our office.
COSMETIC PROCEDURE CANCELLATIONS: We request a 24-hour prior notification for rescheduling or cancellation of all cosmetic appointments, so that we may offer your appointment to other patients. We realize there are sometimes circumstances beyond your control, but we still ask that you communicate with our office. There will be a "No Show" fee of HALF of your quoted treatment price for broken appointments without 24 hours notice or non-communication with our office.
PHONE CALLS: Our office handles a huge volume of phone calls each day. Our staff answers questions about appointments and procedures for new and established patients, obtains new patient information and updates old information. They make and receive calls to confirm, cancel, and reschedule appointments. They forward messages to our office manager, to our billing/insurance specialist, and to our assistant staff regarding patient inquiries, pharmacy prescription renewals, and lab results. In addition, our doctor receive calls from patients, family members, pathologists, and physician colleagues throughout the day. We have an excellent voice-mail system in place to handle messages. Our staff make every effort to return calls by the end of the office session. We request that patients leave accurate contact information (e.g. time, phone number) so they are available for return phone calls during or at the end of office hours. If the problem is urgent or requires prompt attention, the patient should communicate that to the staff member or office manager directly, so the doctor can be informed. We make every effort to accommodate urgent problems as promptly as possible.
EMERGENCY CARE: True dermatologic emergencies are fortunately extremely rare. Most skin conditions can be treated during normal office hours. In cases of true emergencies, patients are advised to go to the nearest emergency room or to see their primary care physician for stabilization and treatment after hours.
FORMS OF PAYMENT: Our office accepts payments in the form of cash, personal checks, debit, American Express, Discover, Mastercard, Visa and CARE CREDIT. A $35.00 return check fee for all returned checks.
